In two sections. First, dealing with seclusion, gives names, dates of placing in and release from seclusion, and reasons for the treatment. Second, dealing with restraint also shows the means of restraint used, its duration, and a certificate from the Medical Superintendent/ Attendant giving the grounds for the treatment. Mechanical restraint was last used in 1932
