Columns generally completed for date of accident, hour of accident, hour of shift, name and address (place only) of injured person, name of colliery, age last birthday, married or single, occupation, place of accident, number of weeks and rate per week for compensation paid, amount of compensation, description of cause of accident.
Including index pages at front, arranged arranged alphabetically by initial letter of surname and by colliery.
