Columns generally completed for name of colliery, name and address of injured person, occupation, age last birthday, married or single, number of children under 15, place of accident, nature of injury, persons present when accident happened - actual and first notified, date of accident, hour of accident, hour of shift, date of first absence, date resumed work, number of weeks off, date claim sent, average weekly earnings, number of weeks and rate per week for compensation paid, amount of compensation paid, date paid up to, description of cause of accident.
Including index pages at front and arranged alphabetically by initial letter of surname.
