Columns generally completed for date of accident, hour of accident, hour of shift, name and address of injured person, name of colliery, age last birthday, married or single, occupation, place of accident, remarks, nature of injury, persons present when accident happened - actual witness and person first notified, date resumed work, number of weeks off, claims sent, average weekly earnings, number of weeks and rate per week for compensation paid, amount of compensation paid, description of cause of accident, date of first absence and date paid to.
Including index pages at front and arranged alphabetically by initial letter of surname.
