Workmen sign to agree to serve the company under the terms of the contract rules printed at front of volume subject to observance of the Coal Mines Act 1911 and regulations.

Columns for date, whether workman has previously suffered from Miners' Nystagmus, whether workman qualified to work alone under Section 73, whether workman received a copy of the abstract and general regulations previous to signing, whether workman has received a copy of contract rules, signature or mark of person employed, age, address, occupation and witness to signature or mark.
