Identified on front cover as record of accidents sustained by workmen employed under the Bolsover Colliery Company Limited, but seemingly only for Bolsover Colliery in effect. Divided into sections for Deep Hard Seam, Jan 1942 -Dec 1945 (pages 1-81 and 113): and Blackshale Seam, Dec 1941-Aug 1943 (pages 81-86). Columns for number, date, shift (time) and hours (number), name (surname with initials), check number, age, occupation, place of accident, nature of injury, time off work, cause of accident, and address (not used). Endboards missing, but front and back pages covered with brown paper.
