Numbered 5 on front. Attached at start, fire examination certificate for South Mill issued by Belper Urban District Council
(i) general information on the factory
(ii) register of young persons under 18 years of age employed full time, with columns for surname, Christian name, residence [place, not detailed], date of first employment, date of labour certificate from Local Educational Authority, names of father and mother, date of birth, signature of certifying surgeon, date of signature [by surgeon], and remarks when young person is 16 and when employment ceased, Aug 1904-Sep 1905, with two incomplete pages from register meant to go next in series (presumably the rest is damaged) for Sep-Oct 1905.
(iii) register of children employed half time, with columns for surname, Christian name, residence [place, not detailed], date of first employment, date of labour certificate from Local Educational Authority, names of father and mother, date of birth, signature of certifying surgeon, date of signature [by surgeon], and remarks when child left or became a young person, Sep 1904-July 1905
(iv) list of dates the factory was limewashed, Mar-Dec 1904
(v) reports of steam engine inspection and examinations, Aug 1904-July 1905.
Labelled 135 on fire certificate. Later part of series numbered O1/212.
