Description
Divided on each page into two sections: (i) particulars prescribed under the Workmen's Compensation Act 1925, with columns for full name, address and occupation of injured workmen, signature of workman or other person making the entry, time and date when entry made, time of accident, date when and place where accident happened, and cause and nature of injury; (ii) voluntary information, with columns for lamp number, age, married or single, number of children dependent, names of witnesses, place to where injured person removed (usually WSO, RTW, home or hospital), and by whom reported.
