Columns for date, whether workman has previously suffered from Miners' Nystagmus, whether workman qualified to work alone under Section 73, age, signature, residence, occupation, number of pit, married or single and witness.
Changed from Pinxton Collieries Limited to National Coal Board 1947, being identified in heading as Langton Unit later Langton Colliery.
Volume has been damaged by damp, several pages at front detached from volume and loose, no front cover.
