Comprising four documents:
Risk Assessment Follow-up Form (FUF), designed to provide feedback on an identified and registered risk area, in this case Preparedness for Flu Pandemic. The document notes additional risks associated with unresolved questions about local coordination of emergency planning, following the abolition of the Primary Care Trust (PCT) and Strategic Health Authority (SHA) bodies.

Joint Major Incident Plan 2012: 54-page document for NHS Derby City and NHS Derbyshire County, sponsored by the interim Director of Public Health, outlining the strategic aims of the Derbyshire Cluster Primary Care Trusts in the event of a major incident and describing the planned operational response.
