Accident in August 1957 not reported until October 1957. Handwritten note states 'Foreign - Dif to understand' (sic). Panel Dr diagnosis was Lumbago and claimant had multiple sick periods with that diagnosis including one previous to the accident that injured back. Doctor's Certificate for NUM said ' a prolapsed intervertebral disc which could have been caused by accident in pit'. Claimant advised : 'go to your doctor immediately and ask if he would be good enough to amend the certificate, or give you a fresh one, more positive'.
