Columns for name of worker; age; address; how employed; date of accident or disablement; name of colliery or department; particulars of how injuries occurred (not filled out until 1913); nature and extent of injuries; details for fatal accident (total amount of earnings on 3 year basis, name and age of dependants, relationship to deceased, level of dependancy); details for non-fatal accident (weekly earnings, weekly payment, dates when payments started and stopped); details if agreement accepted in court (name of court, date of agreement, date recorded); amount of compensation; to whom compensation was paid; date compensation paid; remarks (on whether case settled out of court, by arbitration and how it was otherwise disposed of); where original agreement document filed; company file number (not recorded from 1910).
