This map is described in the Derbyshire Record Society's "A Catalogue of Local Maps of Derbyshire c.1528–1800" (compiled by Harold Nichols, revised by Mary Wiltshire assisted by Susan Woore, published 2012).  The description reads as follows:

A Survey of the Church Lands belonging to Wormhill by I. Bradley [17--]. Surveyor: I.Bradley. Scale: of two chains in an Inch [l:1600] with dividers. Size: 54 x 44 cm. Materials: watercolours on paper. Centred on: SK138733 Notes: compass rose, top of sheet west. Estate plan, 36 acres, showing area north of bridge over river Wye to 'Muxta' (Monk's Dale), brook: closes with reference letters, boundary hedges, fences or walls: 2 gated roads: houses in 2 groups in distinctive elevation, wood: table gives field-names and areas.
